What to Compare

Cash-Pay and Out-of-Pocket Costs

If you're paying out of pocket for GLP-1 medications, these providers offer some of the most competitive rates available. All of them prescribe compounded semaglutide or compounded tirzepatide through licensed clinicians and verified pharmacies. Some also offer oral tablets alongside injectables, giving you more options to discuss with your provider.

What Actually Differs Between Semaglutide Providers

From the outside, semaglutide providers can look similar. They all offer online consultations and ship medication to your door. But the differences matter more than you might think.

Pricing Structure

This is where providers differ most. Some advertise low first-month prices that increase significantly after. Others charge the same rate every month. A few have separate fees for the program and the medication.

When comparing prices, always check:

  • First-month vs ongoing: Is that $179 price just for month one?
  • What's included: Does the price cover medication, consults, and shipping?
  • Hidden fees: Are there charges for lab work, dosage changes, or support?

Level of Medical Support

Some providers focus on getting you medication quickly with minimal interaction. Others build in regular check-ins, coaching, and ongoing support.

Consider what you need:

  • Just the medication: Some people want a straightforward prescription and shipment
  • Guidance and accountability: Others benefit from regular check-ins and coaching
  • Access when needed: Look at how easy it is to reach your care team with questions

Medication Options

Not all providers offer the same medications:

  • Compounded only: Most telehealth providers focus on compounded semaglutide
  • Brand-name available: Fewer providers offer Wegovy® or Ozempic®
  • Tirzepatide options: Some also offer compounded tirzepatide (not FDA-approved)
  • Insurance support: If you have coverage, look for providers that work with insurance

Which Path Fits You?

If you have insurance: Check whether your plan covers Wegovy® or Zepbound® first. If it does, you could pay as little as $25/month with a manufacturer savings card. Providers like WeightWatchers, Ro, and Sesame Care accept insurance and can handle prior authorization on your behalf. You can also call the number on the back of your insurance card to ask about GLP-1 coverage before signing up.

If you're paying out of pocket: Focus on total cost over 3-6 months, not just the first month. A provider charging $249/month with everything included may cost less than one advertising $149 with add-on fees. Our pricing breakdown walks through what to expect at each price point.

If you're not sure where to start: Pick two providers from the comparison above that fit your budget. Complete both free assessments. Compare what each one includes before committing.

Brand-Name vs. Compounded Semaglutide

Most telehealth providers offer compounded semaglutide, not brand-name Wegovy® or Ozempic®. Understanding the difference matters because it affects your cost, your insurance options, and what questions to ask your prescriber.

Important Distinction

Compounded semaglutide is NOT FDA-approved. It has not undergone the same testing as brand-name medications. While compounding pharmacies are regulated, the finished products are not equivalent to FDA-approved drugs.

Brand-Name (FDA-Approved)

  • Wegovy®: FDA-approved specifically for weight management
  • Ozempic®: FDA-approved for Type 2 diabetes (used off-label for weight loss)
  • Cost: $800-$1,400/month without insurance. Manufacturer cash-pay programs bring costs down: NovoCare offers Wegovy® injection at $349/month (with a $199/month intro for new patients on starter doses through March 31, 2026). The Wegovy® pill runs $299/month at maintenance dose, with starter doses from $149/month. LillyDirect offers Zepbound® vials at $299-$449/month depending on dose
  • Insurance: May be covered, especially Wegovy® for weight loss

Compounded (NOT FDA-Approved)

  • Active ingredient: Semaglutide prepared by compounding pharmacy
  • Cost: Typically $150-$600/month depending on the provider and dosage
  • Insurance: Almost never covered
  • Quality: Can vary between pharmacies

Your prescribing clinician will discuss which option fits your health profile during your assessment. If you have insurance, it's worth checking brand-name coverage first. For the full breakdown of how these differ and what to ask, see our brand vs. compounded guide.

Questions to Ask Before You Choose

Before signing up with any provider, get clear answers to these questions:

About Pricing

  • What is the total monthly cost after the first month?
  • What happens to pricing as I move up in dosage?
  • Are there any additional fees (labs, consults, shipping)?
  • What's your cancellation policy?

About the Medication

  • Is this brand-name or compounded medication?
  • Which pharmacy compounds/dispenses the medication?
  • What happens if the medication doesn't work for me?

About Support

  • How do I contact my care team if I have questions?
  • How often will I have check-ins?
  • What happens if I experience side effects?

Bring these questions to your free assessment. A good provider will answer them clearly. If they can't, that tells you something too.

The Right Provider Depends on Your Priorities

There's no single "best" provider for everyone. If you want the lowest price and don't need much support, choose accordingly. If you want more guidance, pay for it. The key is matching your needs to what each provider actually offers.

Pick 2-3 providers from the comparison above. Check their pricing page for answers to these questions. Then complete a free assessment with the one that fits. Our provider selection guide walks through the full decision process.

Common Questions When Comparing Providers

Providers have different pricing models. Some include everything in one price. Others charge separately for the medication, consultations, and support. Some use low first-month prices as promotions.

When comparing, look at the total cost over 3-6 months, not just the first month. Also check what's included. A higher-priced provider might actually be better value if they include more support. Our semaglutide pricing page breaks down what drives cost differences.

Price matters, but it's not everything. Consider:

  • Support needs: Do you want coaching and check-ins, or just medication?
  • Medication type: Do you need brand-name or is compounded acceptable?
  • Flexibility: Can you pause or cancel easily?

The cheapest option isn't always the best fit. The most expensive isn't either. Match your needs to what each provider offers.

Yes. You're not locked in. Most providers allow you to cancel anytime (before your next shipment). You can then sign up with a different provider.

Keep in mind you'll need a new evaluation with the new provider. Your prescription from one provider doesn't transfer to another.

No. While most offer compounded semaglutide, there are differences:

  • Some offer only semaglutide, others also offer tirzepatide
  • Few online providers offer brand-name Wegovy® or Ozempic®
  • Compounded formulations can differ between pharmacies

If you have a preference for a specific medication, check what each provider offers before signing up.

Start with these three steps:

  1. Call your insurer. Ask if GLP-1 medications for weight management are covered under your plan, and whether you need prior authorization.
  2. Check provider options. Look for telehealth providers that accept insurance and can handle the prior auth process for you.
  3. Compare cash-pay as a backup. If insurance doesn't cover it, or the prior auth takes too long, compounded options through a telehealth provider are typically $150-$600/month out of pocket.

Brand-name Wegovy® with insurance and a manufacturer savings card can cost as little as $25/month. Without insurance, NovoCare offers Wegovy® injection at $349/month ongoing (with a $199/month intro for new patients on starter doses through March 31, 2026). The Wegovy® pill starts at $149/month for starter doses, with the maintenance dose at $299/month. LillyDirect offers Zepbound® vials from $299/month. Our pricing breakdown covers the different paths and what you can expect to pay for each.
